Diabetes Care within the Practice.

We aim to support people with diabetes within the practice, working closely with our secondary care colleagues when this is appropriate.

We offer at least an annual appointment, many people will be seen more often depending on how well you are and whether we are adjusting medication/treatment plans. Ideally, bloods are taken prior to the appointment so the results can be discussed

We will be looking closely at glucose levels but also blood pressure, cholesterol levels, urine results and kidney function. 

All people with diabetes will be referred to the Retinal Screening Service (for regular eye checks). This is in addition to any care you receive from your local optician. This service is offered by the hospital, and you can be offered an appointment anywhere within our locality. You can contact the Retinal Screening Service to ask for a closer appointment if the one you are offered is some distance away. 

We will ask you to bring a urine sample to your appointment at least once a year. Ideally an early morning sample (the first one you pass). 

We will ask to look at your feet at least once a year and offer advice and support regarding foot health. 
We offer a range of treatment options aiming to provide you with safe and effective treatment, whilst supporting lifestyle changes which will support your diabetes health. 

When your review is due you will be contacted by text or letter and invited to make an appointment. There are no set days/times for reviews.
