Joined-up Services

Social prescribing enables GPs and other health professionals to refer patients to a range of non-clinical services through a link worker.  We have a social prescriber working in the practice for two days each week. This holistic approach of connecting people with community groups and other services enables practical and emotional support to be provided and helps people with their health and well being. It works for a wide range of people including those:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing

Are you aged 11-18 or up to 25 with SEND and registered with an East Riding GP?

Your Social Prescribing Link Worker will focus your support based on what matters to you.

Referrals are made by any member of staff in your GP practice through a referral form

If you are experiencing one or more of the following then we can help you:

  • Low self esteem
  • Feeling isolated
  • Low mood
  • Wanting a healthier lifestyle
  • Low level mental health difficulties
  • Young carer in need of support
  • Struggling to make friends
