Social prescribing enables GPs and other health professionals to refer patients to a range of non-clinical services through a link worker. We have a social prescriber working in the practice for two days each week. This holistic approach of connecting people with community groups and other services enables practical and emotional support to be provided and helps people with their health and well being.
